Our Mission

Aviana Health Care will provide care and support to individuals in their own home in a manner the service user finds most agreeable. We have sound principles for the way we provide our services. Central to these, is our belief that the rights of service users are paramount, and their wishes and aspirations inform and shape service delivery by a highly skilled and dedicated staff team.

Aims

We aim to equip our service users with the knowledge, skills, and ability, to take as fuller control of their lives as possible; thereby enabling them to feel more fulfilled in life through the range of opportunities offered to them.

Our objective is to:

  • To provide the service user with a safe, consistent, respectful, high standard of care and support within their own home in a way that is agreed by them
  • Assist the Service User to maintain his/her quality of life and to remain in their own home for as long as possible and as long as it is in the Service Users own interests
  • Provide a flexible, efficient, value for money quality domiciliary care, live-in care and support service
  • To provide tailored, person centred holistic care to all service users, taking into account their wishes and needs and bearing in mind family and informal carers’ needs
  • Assist our service users to develop their own individualised care/support plan in full consultation with family, friends and care professionals
  • To promote a culture of continual learning within the staff team and foster continual improvement in service delivery
  • To provide all staff with relevant, high standard, quality induction and annual update training and to encourage ongoing personal
  • To provide experienced and committed staff that have the appropriate expertise and training to provide a holistic service to adults with a range of complex needs such as:
  • To enable vulnerable adults to develop their confidence and maintain their dignity, by retaining control of the care and support services they receive
  • Empower our service users by enabling them to choose their care or support workers
  • Involve users of our services in the recruitment and selection of care and support staff
  • Provide opportunities to choose from the range of recreational, educational and employment opportunities that are available in the community
  • Safeguard the privacy and confidentiality of the Service User, to enable Service Users to maintain their personal independence, have informed choice and an opportunity to enjoy and contribute to society
  • To deliver a sustainable domiciliary care service based on good practice, which is responsive to market demands
  • To improve outcomes for service users by facilitating proactive solutions to a number of care and support related
  • Work in a way which promotes integrated working with health partners and facilitates new models of care by utilising assistive technology to full effect and reducing dependency wherever possible

  • Will benefit from the knowledge that the Care Quality Commission is informed of the services being provided by Aviana Health Care
  • Aviana Health Care, by complying with the regulations will:
  • Have a statement of purpose that is kept under review, and give a copy to the Care Quality Commission
  • Notify the Care Quality Commission of any changes to their statement of purpose
